Referrals can be made by individuals, families, hospitals, support coordinators, health professionals and community organisations.

Our referral process

We aim to make the referral process as simple and straightforward as possible. Whether you are a participant, family member, support coordinator, allied health professional or another service provider, we are happy to discuss how we may be able to support you.

1: Submit a referral

Complete our referral form below with as much information as possible.

2: We review your referral

Our intake team will review the information provided to understand your needs and determine whether we have the appropriate services, staff and experience to meet your needs.

3: We get in touch

We’ll contact you to discuss goals, support needs, funding, location and preferred supports. This gives us an opportunity to better understand what you are looking for and whether we are the right fit.

4: We discuss the next steps

If we are able to provide the required support, we’ll work with you and your support network to develop a person-centred plan that works for you and identify the right people for your support team.
